The lamp itself appears to be in new condition with no visible damage to the glass envelope, filament, or G17q four-pin base. The original Sylvania Blue Dot box is included, though it shows significant age-related wear: the cardboard is creased, torn at corners and edges, and shows discoloration consistent with long-term storage. This is new old stock that has been in storage for an extended period. The lamp is the correct Sylvania CYS-DBH type as printed on both the blue protective cap and the box label.
